Trinity Health (Livonia, Mich.). Trinity Health includes 93 hospitals and 121 additional home care, continuing care and outpatient facilities across 22 states. The Catholic health system includes 131,000 employees and provides around 2 million visits annually. The health system formed in May 2013 as the result of a merger between Trinity Health and Catholic Health East, formerly based in Newtown Square, Pa. The system now reports operating revenue of $17.6 billion and assets of $23.4 billion. In 2016, CMS chose Trinity Health to participate in the Next Generation ACO Model building on the Pioneer ACO Model and Medicare Shared Savings Program.
