Surescripts’ Incorrect Medication Reconciliation Data May Create Safety Issues

Medication reconciliation provider Surescripts has announced some data in its Medication History Acute and Medication History Ambulatory may contain inaccurate information, which could cause patient safety problems, according to a National Alert Network report.

The potential problem stems from missing characters, such as decimals, slashes and percentages in records having to do with drug strength, causing potential drug strength statements off by orders of magnitude, according to the alert.

Surescripts has disconnected the questionable data sources until the problem is corrected. It is also conducting an internal review of its other data, though it says there is no reason to believe records relying on any others of its data sources are inaccurate. The company suggests providers double-check any medication strengths they might reference.
