Report: U.S. Third-Worst Among 12 Developed Countries for Value-Based Healthcare

A new report on the state of healthcare in 12 developed countries shows that the U.S spends the most money on healthcare, but ranks near the bottom of the list in terms of implementing value-based care — controlling costs by improving health outcomes while holding or lowering the cost of care.

Boston Consulting Group — which issued the new report as a follow-up to a December 2011 article in Health Affairs on the effective use of disease registries among 13 disease registries in five countries — assessed the progress of 12 developed countries in adopting value-based care. The U.S. was the third-worst among the countries evaluated.

The report found the fragmented nature of the U.S. healthcare system limits collection of national health-outcomes data and standards, and clinical outcome metrics differ widely across the nation. Further, there is no national mechanism to compel providers to report outcomes to disease registries.

According to the report, Sweden is the most advanced country of the 12 studied in implementing value-based care, followed by Singapore, Canada and the United Kingdom. These countries are succeeding in providing value-based healthcare because they have:

  • Common national standards;
  • An information technology infrastructure;
  • National legal and consent frameworks;
  • The ability to link health outcomes with costs;
  • High engagement on the part of clinicians and policymakers; and
  • Strong national disease registries.
