Patient Safety Tool: General CUSP Toolkit

The Agency for Healthcare Research and Quality offers complimentary resources designed to help healthcare facilities eliminate healthcare-acquired infections.

The General CUSP Toolkit for comprehensive unit-based safety is an instructional resource for unit leaders to teach team members about understanding safety and incorporating safety considerations into daily operations.

The toolkit contains six modules: understanding the science of safety, assemble the team, engage the senior executive, identify defects through sensemaking, implement teamwork and communication and apply CUSP.  Each module contains facilitator notes, slides, videos and tools for customization and unit use. AHRQ Recommends new CUSP users complete the modules in order.
