31 Statistics on Preventable Hospitalizations by State

Here is a ranking by state of the number of potentially preventable hospitalizations related to chronic disease per 100,000 adults, from least preventable hospitalizations to most, according to information from the Robert Wood Johnson Foundation's DataHub.

Chronic diseases considered include diabetes, congestive heart failure, hypertension, angina, chronic obstructive pulmonary disease and asthma.

Data comes from state health access data assistance center analysis of 2009 Healthcare Cost and Utilization Project data from the Agency for Healthcare Research and Quality.
