Helen Haskell. President of Mothers Against Medical Error. Ms. Haskell has sought to improve patient safety and quality after her son, Lewis, died from a medical error in 2000. She serves on the board of directors at the National Patient Safety Foundation, Consumers Advancing Patient Safety and the Institute for Healthcare Improvement, among others. Ms. Haskell took an active role in implementing patient safety legislation in her home state of South Carolina and recently retired from her role on the Agency for Health Research and Quality’s National Advisory Council.
