The hospitals were fined $25,000 for each violation as part of an effort to hold hospitals accountable for placing patients at serious risk for death or injury. The disclosures of these incidents are the result of a state law that took effect in 2007 that requires hospitals to inform health regulators of all substantial injuries to their patients, according to the report.
As a result of the violations, most of the hospitals that were fined have taken corrective action and are examining and revising their policies.
