Dr. Feldman co-presented with Patricia Metzger of Orange Regional Medical Center at the annual meeting in New York. Together the duo discussed Orange Regional Medical Center and how to use tools to ensure safe transitions from facility and home, according to a press release.
Dr. Feldman noted common points where hospitals have break downs in their systems:
– Medication
– Transistion to primary care physician
– Patient self advocating
– Recognizing signs and symptoms that are red flags
– Willingness and ability to call for help
– Pyschosocial issues
– Transportation
– Support system
