Temple University Health System Creates Institute for Population Health

Philadelphia-based Temple University Health System has created the Institute for Population Health to improve the health of urban populations in Philadelphia, increase healthcare quality and lower costs.

The institute will partner with primary care practices, federally qualified health clinics, city health clinics and others to facilitate communication between providers when patients are admitted to Temple’s emergency department or hospital, according to the news release. The institute will also work with acute rehabilitation units, sub-acute nursing facilities, community hospitals, nursing homes and long-term acute-care hospitals to integrate care.
