Post-Acute Physician Partners will establish a multi-state network of primary care physicians, specialists, nurse practitioners and physician assistants who will work in existing facilities. The teams will focus full-time on decreasing complications, overseeing medication management and smoothing transitions after the patient leaves the hospital for a rehabilitation home, nursing home or their own home, according to the report.
