Carroll Health Group in Maryland Receives PCMH Recognition

Manchester, Md.-based multispecialty physician group the Carroll Health Group has received patient-centered medical home recognition from the National Committee for Quality Assurance, according to a Carroll County Times report.

Carroll Health Group is among more than fifty other multi-payor PCMHs in Maryland.

To be recognized as a PCMH, practices need to offer expanded office hours, use data to identify and manage patient populations, enhance care management through evidence-based guidelines, support a self-care and family care process for patients, track referrals and follow-ups as well as use data to manage ongoing quality improvement.
