The worksheets will assess compliance with quality assessment and performance improvement, infection control and discharge planning. This patient safety initiative aims to reduce hospital-acquired conditions, including healthcare-associated infections, as well as preventable readmissions.
CMS may make additional changes to the worksheets during the pilot phase, which is scheduled to end on an unspecified date in fiscal year 2013.
