Baptist Health System Begins Health Coach Program to Reduce Hospital Admission, Readmission

 
Birmingham, Ala.-based Baptist Health System has started employing registered nurses as health coaches to ensure that chronic condition patients are taking the necessary precautions to prevent hospital admission or readmission, according to a Birmingham Business Journal report.

The health coaches will call patients, upon referral from physicians, to assess their needs and determine how frequently they may need follow up. “We are the extension of the physician after the patient leaves their office,” said Patricia Poe, RN, on of the hospital’s health coaches.
