A resident-led quality improvement initiative at a multispecialty teaching hospital in central Texas studied same-cause readmissions — readmissions within 30 days of discharge for the same condition diagnosed during the initial hospital stay. Specifically, the team examined readmissions to the hospital’s internal medicine service.
Phase I results showed the following patients may be at a higher risk of same-cause readmissions:
1. Patients diagnosed with chronic obstructive pulmonary disease/asthma or anemia
2. Medicaid beneficiaries
3. Patients needing assistance from social services
