In its investigations, the CDPH found various patient safety issues at the hospitals, such as:
- Improper discharge planning leading to a patient being abandoned on the side of the road waiting for a ride
- Allowing a patient’s family access to a device dispensing narcotics, which led to the patient receiving 19 doses in a short amount of time who then died nine days later
- Improperly supervising residents, allowing one resident to insert a feeding tube into a patient’s lung instead of the stomach, leading to the patient’s death
- Giving a patient a vaccine against physician orders, which resulted in a serious eye infection and blinding of the patient

4. Loma Linda University Medical Center — $50,000
5. Mark Twain Medical Center (Escondido) — $50,000
6. Palomar Medical Center (Escondido) — $50,000
7. Rideout Memorial Hospital — $50,000
8. Southwest Healthcare (Murrieta) — $100,000
9. UCSF Medical Center (San Francisco) — $100,000
10. UC San Diego Medical Center — $100,000
Hospitals are required to provide a plan of correction to CDPH to prevent future incidents. They can appeal their penalty by requesting a hearing within 10 calendar days of the notification.
