Forecast Health, a health IT company focused on predictive analytics, uses clinical, claims and socioeconomic data to identify patients at risk for adverse events like readmissions or postoperative complications.
Forecast Health also determines which patients will respond best to certain interventions, so that Mission Health Partners can target its resources with patient-specific guidance.
