Humana's population health arm launches wellness solution suite

Transcend Insights — a population health company owned by Humana — is rolling out a new unified solution suite, built on its technology platform HealthLogix.

The solution suite expands upon Transcend Insights' existing population health platform for community-wide interoperability and real-time analytics. The new suite is designed to help health system executives and care teams transition patients from episodic to proactive care for long-term wellness.

The suite includes a performance measurement application to evaluate quality and utilization metrics, a tablet and web-based application to give a comprehensive view of patients' claims and wellness data, and a patient engagement solution.

"Our vision for population wellness is to disrupt the 'sick-care cycle' that represents the majority of healthcare delivered today," said Marc Willard, president of Transcend Insights. "In most contexts, population health refers to managing care for the sick patient — wellness is not a part of the equation. We see this as a vastly untapped opportunity to make an impact on people's lives and their health."

According to Mr. Willard, the company can address health and wellness with its HealthLogix platform, by connecting disparate systems with open standards and making it easier for care teams to do their jobs and deliver a better care experience.

Transcend Insights will be presenting the new solution suite at the 2016 HIMSS Annual Conference and Exhibition in Las Vegas.
