BayCare taps Cerner, Salesforce, Lumeris to improve senior care

Clearwater, Fla.-based BayCare Health System is joining forces with Cerner, Lumeris and Salesforce to better manage its senior patient population.

BayCare will implement Cerner's population health platform HealtheIntent, Salesforce's customer relationship management technologies and Lumeris' value-based care services to increase its senior patients' access to care.

"These technologies, processes and behaviors provide our clinicians with data, insights and improvements to their workflow that help us engage with patients and proactively manage their health and well-being," Tim Thompson, senior vice president and CIO of BayCare, said in a news release. "By working with these three industry leaders, we are able to engage and support our seniors to identify gaps in care and work to improve their health."

Here's how BayCare plans to use each company's services, as described in a Cerner news release:

  • Cerner: "BayCare is using Cerner's big data and insights platform for population health management to analyze medical data across platforms and support clinicians in achieving actionable insights to help engage people beyond the hospital setting and manage health outcomes."
  • Lumeris: "BayCare is working to provide an optimal patient experience and better outcomes for seniors enrolled in the BayCarePlus Medicare Advantage health plan through their strategic operating partner, Lumeris"
  • Salesforce: "With Salesforce, BayCare can personalize communications at scale to deliver pertinent information at the appropriate time to support engagement and care plan adherence goals."
