Ten medications accounted for about 28.6% of the $100 billion in 340B outpatient drug purchases in 2025, according to new data from HRSA released July 13. The figure comes as 340B purchases hit a record $100 billion for the year, continuing a stretch of double-digit annual growth for the program.
Oncology drugs claimed four of the top 10 spots, consistent with the therapeutic area’s role as one of the primary drivers of national drug spending growth in 2025.
The brand names of the 10 drugs with the highest expenditures in the 340B program last year were:
- Keytruda (oncology): $8.89 billion
- Biktarvy (HIV): $4.76 billion
- Darzalex Faspro (oncology): $3.12 billion
- Opdivo (oncology): $2.39 billion
- Ocrevus (multiple sclerosis): $1.93 billion
- Mounjaro (diabetes, weight management): $1.75 billion
- Imfinzi (oncology): $1.49 billion
- Descovy (HIV): $1.49 billion
- Trikafta (cystic fibrosis): $1.49 billion
- Vyndamax (cardiology): $1.28 billion
