340B purchases reach record $100B: 5 things to know

340B covered entities purchased $100 billion in outpatient drugs in 2025, according to new data from HRSA released July 13. It marks the program’s largest annual total on record, continuing a run of year-over-year growth from $53.7 billion in 2022 to $66.3 billion in 2023 to $81.4 billion in 2024. 

The milestone comes as the program faces mounting pressure from drugmakers over claims-data reporting requirements. Eli Lilly has pushed hospitals to submit claims-level data or lose 340B pricing, a policy several hospital associations have called unlawful. Tampa General Hospital sued Lilly over the resulting price increases, becoming the first hospital to challenge the enforcement push in court, part of a broader wave of litigation and policy shifts reshaping the program this year. 

Here are five things to know about the new purchase data:

1. Disproportionate share hospitals accounted for the largest share of purchases at $79.2 billion, followed by health center programs at $5.9 billion and sexually transmitted disease clinics at $2.9 billion.

2. Children’s hospitals purchased $2.5 billion in covered outpatient drugs and rural referral centers purchased $2.5 billion, rounding out the top entity types by spend.

    3. High-cost pharmaceuticals purchased through specialty distribution channels made up just 38.1% of all 340B units purchased but accounted for 61.9% of total purchases, or $61.9 billion.

    4. Oncology drugs, biologics and other infusion therapies administered in hospital outpatient departments represent the fastest-growing segment of 340B purchases, as care increasingly shifts from inpatient to outpatient settings. See the full list of the top 10 340B drugs purchased here

        5. The 340B growth mirrors national trends. Overall US payer net spending on prescription drugs grew 9.08% in 2025 to $901 billion, with oncology, immunology and obesity drugs as primary growth drivers.
