The pharmacy can and should be a key contributor to reducing unnecessary readmissions, improving patient satisfaction scores, improving medication adherence and ensuring seamless transitions of care – as well as drive meaningful savings. A 2017 study published in Integrated Pharmacy Research and Practice found that “pharmacists can contribute to substantial healthcare savings across a variety of settings.”
