A pharmacy has been fined $1 million by Pennsylvania’s Department of State and placed on probation for compounding and distributing tens of thousands of doses of injectable weight loss drugs in uninspected facilities.
Kennett Square-based Boothwyn Pharmacy began producing GLP-1 medications in September 2024 at its main building in Kennett Square, where staff compounded more than 1,000 doses per day, according to an Oct. 21 news release from the Department of State. The medications were then transferred to a second, unlicensed building for inspection, verification and packaging — a facility that had never been approved for such use by the State Board of Pharmacy.
The Department of State conducted four inspections between November 2024 and March 2025 and said pharmacy staff failed to disclose the ongoing compounding activities or the second building’s use as a processing site.
In a consent agreement, Boothwyn Pharmacy admitted to violating the Pharmacy Act and State Board regulations. As part of the disciplinary action, the pharmacy placed $500,000 of the civil penalty in escrow and must pay the remaining $500,000 within one year. The pharmacy must also obtain proper inspection approvals and accreditation for its compounding operations before it can request the removal of its probationary status.
