Humana signs value-based agreement with 248-member physician group

Louisville, Ky.-based Humana added Community Care Physicians in Latham, N.Y., to its value-based Medicare Advantage network.

The agreement will allow Humana Medicare Advantage members in-network access to CCP’s 248 physicians across 37 practices. The practices are in Delmar, Niskayuna, North Greenbush, Albany and Latham, all in New York state.

Humana and CCP will share data analytics tools and coordinate care to improve health measures while lowering costs. CCP physicians’ reimbursement will be tied to members’ health outcomes.
