Philadelphia-based Einstein Healthcare Network entered into a value-based agreement with Humana.
Under the agreement, which expands on the parties' previous contract, Humana's Medicare Advantage members will maintain in-network access to Einstein Healthcare Network facilities and providers. The value-based component will team Humana with Einstein Healthcare Network's ACO, called Einstein Care Partners.
The value-based payment model will emphasize improved care for members with chronic conditions, use data analytics to coordinate treatment and reimburse physicians based on patients' health outcomes.
