Clover Health platform to link kidney disease patients with Cricket Health clinicians

As part of an effort to expand renal care coordination, tech company Clover Health is entering a value-based agreement with San Francisco, Calif.-based specialty kidney care provider Cricket Health.

Announced Dec. 9, the agreement allows clinicians using the payer's patient data platform, Clover Assistant, to connect patients with chronic kidney disease to Cricket Health's clinicians, mentors and and social workers at no additional cost. 

The platform taps patient data to analyze kidney disease status and progression and guides clinicians through care. 

"We created the Clover Assistant to give primary care physicians superpowers. So what better way to do that than give them access to innovative healthcare service providers like Cricket Health?" said Andrew Toy, Clover Health president. “By continuing to integrate best-in-class clinical support services for conditions like CKD into the Clover Assistant, we are elevating the level of care delivered to our beneficiaries.”
