Boulder Medical Center, Humana roll out value-based payment model

Boulder (Colo.) Medical Center and Humana launched a value-based healthcare delivery model for Humana Medicare Advantage policyholders in Colorado.

Core components of the model include a focus on preventive care, chronically ill patients and coordinating physician care through data analytics.

Under the model, Louisville, Ky.-based Humana’s Medicare Advantage members gained in-network access to Boulder Medical Center’s 80 physicians across four locations. Humana’s commercial policyholders previously had in-network status with the physicians.
