If the survey is completed within 90 days of enrollment, eligible policyholders will receive a $10 incentive on their Anthem Rewards cards to apply toward most purchases at participating Wal-Marts. New members of Anthem Blue Cross and Blue Shield’s Hoosier Healthwise, Healthy Indiana Plan and Hoosier Care Connect plans in Indiana are eligible.
Current Anthem Medicaid policyholders can take the risk assessment online or over the telephone.
