10 States that dominate the SMB payer market

The small and medium-sized (SMB) health plan market has a broad geographical footprint.

A recent report on the SMB Health Insurance Market revealed that state-level distribution of all 409 SMB plans (defined as those with fewer than 50,000 lives and between 50,000 and 499,000 lives, respectively) is relatively proportionate to state population, with the more populated states having more SMB payers.

California, the state with the highest population, has the most SMB payers (62), while Alaska, the least populated state, has the fewest SMB payers (11). However, there are two states that make the list of top ten states with the highest number of SMB payers that are not in the top 10 when it comes to population: Wisconsin and Oregon. Wisconsin ranks 20th by population in the U.S. but 4th in the Top 10 SMB payer states, while Oregon ranks 27th by population and 8th in the Top 10 SMB payer states. In these two states interestingly enough, there is a much higher proportion of SMB payers compared to overall population numbers.

A recent infographic from Edifecs ranks the top 10 states by number of SMB payers in 2018 as follows:
1. California (population of 39 million) - 62 SMB payers
2. New York (population of 19 million) - 48 SMB payers
3. Texas (population of 28 million) - 41 SMB payers
4. Wisconsin (population of 5 million) - 37 SMB payers
5. Pennsylvania (population of 12 million) - 35 SMB payers
6. Florida (population of 20 million) - 33 SMB payers
7. Oregon (population of 4 million) - 33 SMB payers
8. Illinois (population of 12 million) - 32 SMB payers
9. Michigan (population of 9 million) - 32 SMB payers
10. North Carolina (population of 10 million) - 31 SMB payers

The infographic also outlines the rate of growth over the last five years. Only one state, Florida, out of all ten states has seen a decrease (15 percent) in the number of SMB plans since 2013.
