1. Texas - 1,165 fraud investigations
2. Indiana - 812 fraud invetigations
3. California - 718 fraud investigations
4. Ohio - 701 fraud investigations
5. New york - 677 fraund investigations
According to the annual report, the following five states had the most criminal and civil Medicaid fraud recoveries for 2013:
1. Virginia - $1billion
2. Texas - $196 million
3. Louisiana - $187 million
4. New York - $134 million
5. Tennessess - $78 million
