The two locations that closed were Fort Edward (N.Y.) Medical Center and Broad Street Internal Medicine in Glen Falls, according to the report.
The state health department regulates and approves closure to ensure continuity of care for patients. The hospital submitted a closure plan to the state that included notifying The Post-Star to publicize the closures and help alert patients. The newspaper did not receive notification of the medical office closures, according to the report.
The hospital network has lost 40 percent of its primary care providers in the last year and has been downsizing and moving practices, according to the report.
