CMS invited states to apply for the demonstration project, and officials anticipate including up to six states, whose projects would be evaluated independently under the initiative.
This project is the first time Medicare, Medicaid and private insurers will partner with states to promote medical homes. To be eligible, states must be able to meet certain requirements, including having a state agency to administer the program, being able to pay participating practices six months after selection and having mechanisms to connect patients to community health resources, according to a news release.
