The PCMH program focuses on the health needs and care coordination of individual patients. Practices in the program offer 24-hour access to members of a care team and work with community-based services such as nutrition counseling, home care and chronic disease management.
“Our teams worked incredibly hard within each practice to help them achieve the PCMH designation,” Paul MacLellan, CEO of Medical Advantage Group, said in a statement. “We’re proud of the work we’re doing to help physicians transform their practices and improve the level of care provided to their patients.”
The 176 practices account for more than 318 physicians combined.
