Arris Veronie, CEO at the hospital, Diane Carugati, vice president, and Marc Rothman, chief medical officer, were all released from their positions at Friends, according to the report.
The regulatory investigations were brought about after the suicide of a patient who had been seen at the hospital’s emergency room the night before. The report also said that allegations of “inappropriate sexual behavior” and general security and monitoring issues have been brought against the hospital.
Mr. Veronie previously denied that there were any problems regarding the quality of care at Friends, according to the report.
In May, Philadelphia officials told the hospital that they would stop sending Medicaid patients to Friends’ crisis center as of July 1.
Tennessee-based Psychiatric Solutions, which operates Friends, has hospitals in other states that have also been accused of mismanagement, according to the report.
