WellCare targets chronic illness with home telemonitoring program

Tampa, Fla.-based payer WellCare will pilot a home telemonitoring initiative for chronically ill Medicare Advantage members in three states.

The pilot will include up to 500 members with diagnosed congestive heart failure and diabetes in Tennessee, Arkansas and Mississippi. Remote 24/7 telemonitoring will permit physicians real-time access to patient vitals, allowing physicians to immediately respond to any complications and reduce readmissions. 

WellCare serves 98,000 Medicare Part D and 41,000 Medicare Advantage members throughout the three states included in the pilot.
