The Monroe, N.Y.-based federally qualified health center will transition all 36 of its providers onto the eClinicalWorks platform for both primary care and behavioral health services.
Ezras Choilim will also implement eClinicalWorks’ population health solution and its population care planning for behavioral health tools to facilitate care coordination and engage patients in preventative care.
