The pilot will build an integrated, countywide health system that connects care services for roughly 50,000 residents dealing with homelessness, mental illness or substance abuse. The department has selected Eccovia Solutions’ ClientTrack Care Coordination solution — an integrated care coordination platform — for the pilot program.
The ClientTrack Care Coordination platform will connect primary care and community service organizations, in an effort to integrate social determinants of health into healthcare decisions. The platform will connect 11 local programs, including homeless care and substance abuse disorder centers.
