Los Angeles County Department of Health chooses Cerner’s population health management platform

The Los Angeles County Department of Health Services has implemented Cerner’s population health management platform, HealtheIntent.

LACDHS’ Cerner implementation is in collaboration with the California PRIME Project. LACDHS, the second-largest U.S. municipal health system, is Cerner’s 100th HealtheIntent client.

HealtheIntent aggregates patient data from numerous sources. By pairing HealtheIntent with HealtheRegistries and HealtheEDW, Cerner’s registries, scorecards and enterprise data solutions, physicians can identify gaps in care and analyze population health metrics.

Cerner currently supplies the EHRs for Los Angeles County’s four acute care medical centers — West Carson, Calif.-based Harbor-UCLA Medical Center, Los Angeles-based Los Angeles County + USC Medical Center, Los Angeles-based Olive View-UCLA Medical Center and Downey, Calif.-based Rancho Los Amigos National Rehabilitation Center.
