Hospitalists of Arizona, a medical practice group, was informed of the data breach on Jan. 15, and immediately notified St. Mary’s Hospital security and local law enforcement.
The stolen laptop contained the affected patients’ names, dates of birth, medical record numbers and prescription information.
Hospitalists of Arizona has been working with authorities to investigate the laptop theft, according to the report.
In response to the data breach, Hospitalists of Arizona has offered affected patients one year of free credit monitoring services.
