The operating system includes applications to integrate clinical and claims data from various EHR and billing systems and to process disease registries and post-discharge care plans for care coordination. The system also has tools that work on population health management projects, by identifying and stratify at-risk populations.

Innovaccer hopes these data tools will help healthcare organizations streamline their clinical and financial processes in an effort to ease transitions to value-based care.
