Innovaccer’s platform integrates data from EHRs, medical claims and lab systems to help hospitals, ACOs and other healthcare facilities extract actionable patient insights. With its new AI tool, healthcare organizations can use Innovaccer to send out surveys to patients that will help evaluate their social needs.
Social determinants of health include an individual’s access to food and housing as well as economic conditions. An estimated 68 percent of patients experience challenges in at least one category, according to a survey from revenue cycle technology provider Waystar.
Once a patient’s social determinants of health are identified, Innovaccer’s AI tool will provide referrals to community resources that can assist with meeting the patient’s needs. The system also sends automated and real-time alerts to providers if a patient’s needs are urgent. All patient insights are integrated with the healthcare organizations’ EHR systems.
