The award honors organizations that have used health IT to improve patient outcomes.
This year, HFMG, which includes more than 100 physicians in 40 locations, completed three major accomplishments that contributed to the award. HFMG reduced 30-day readmissions by 30 percent, reduced high alcohol consumption by 18 percent for at-risk patients and increased provider time with patients by 27 percent for 15-minute appointments.
HFMG will be honored at the 2017 HIMSS Conference in February 2017 in Orlando, Fla.
