Wellbridge Health will provide select Gateway Health members with iPhones and monitoring devices, which will collect data for daily monitoring. By providing timely behavioral and clinical interventions, the two organizations hope to reduce hospitalizations and emergency room visits. Because the program is targeted at Medicaid and Medicare plan members, Wellbridge Health stresses that their technology also provides insight into social dynamics.
“Poverty, isolation, cognitive issues and depression are often barriers to self-management,” said Cynthia Zydel, CEO of Wellbridge Health. “Our frequent interaction and proactive interventions address these barriers and reduce unnecessary hospitalizations and emergency room visits.”
