The partnership between Gary Comer and SNC aims to advance health information technology that improves care coordination between providers. The addition of the vCare platform helps care managers engage the patient in their own care.
The vCare platform allows care managers to communicate with patients via email, a smartphone app, text, video chat or a telephone call.

The vCareConnect acquisition strengthens our mission of increasing access, quality, and health equity through the innovative use of technology,” said Chris Cruttenden, president of Safety Net Connect, in a statement.
