Most health systems haven’t left on-prem IT: KLAS

More than 80% of healthcare organizations are using a public cloud provider; however, few have fully transitioned their IT infrastructure away from on-premises systems, a July 22 report from KLAS Research found.

KLAS’ report includes responses from 36 health systems and hospitals]across care settings. Respondents were interviewed between April 2024 and April 2025 to assess the current state of their IT infrastructure, experience with public cloud providers, future plans, and general infrastructure priorities and roadblocks.

Here are five key findings from the report:

  1. Microsoft Azure is the most commonly used primary cloud provider among respondents, followed by Amazon Web Services.

  2. Google Cloud Platform is used by only a small number of organizations and has a limited presence in the healthcare space.

  3. Only one of the 36 healthcare organizations surveyed has fully migrated its infrastructure to the cloud.

  4. Nearly 40% of respondents reported that at least 90% of their IT systems remain on-premises.

  5. Many organizations plan to accelerate their cloud migration within the next one to two years.
