The three incidents affected a total of 15,537 patients. While not much information is available on the cause of the data breach, the HHS Office for Civil Rights’ portal indicates the incidents were reported Aug. 15.

Community Psychiatric Clinic is expected to merge with Sound, a Washington mental health and addiction treatment provider, in the fall of 2019. Sound did not return the HIPAA Journal’s request for comment.
