For the report, Beazley examined the major causes of data breaches that healthcare organizations in the U.S. reported to the company during the first nine months of 2017.

Here are top six causes of healthcare breaches thus far in 2017, as reported to Beazley.

1. Unintended disclosure: 41 percent

2. Hack or malware: 19 percent

3. Insider: 15 percent

4. Physical loss: 8 percent

5. Portable device: 6 percent

6. Social engineering: 3 percent
