Through a three-year, $320,538 grant from the U.S. Department of Agriculture, Mountain Home, Ark.-based Baxter Regional Medical Center plans to reduce its readmission rate by implementing a new telehealth system, reports The Baxter Bulletin.
The system, Complex Care Management, will monitor patients with a high risk of readmission. Each patient will receive a kit placed in their home. Through Bluetooth technology, Baxter will be able to track patients' progress. All relevant information will be sent to a portal at the hospital, where a staff member will keep an eye on each patient's health.
The telehealth technology will not replace in-person visits.
