“The printing error did result in the disclosure of your child’s name and unique Affinity member identification number, along with your address,” the notification letter reads. “However, we want to assure you that none of your child’s medical or health information was disclosed.”
Representatives of AHP are reassuring members that the information disclosed cannot be used to access any additional information about members or their children. They are asking all members who mistakenly received letters to securely dispose of them.
