5 recently launched hospital-at-home programs

Here are five health systems that launched “hospital-at-home” programs since February:

  • Edison, N.J.-based Hackensack Meridian Health announced plans Feb. 3 to launch a hospital-at-home program at the JFK University Medical Center, also based in Edison. The program will provide acute care at home to Medicare patients, and includes services such as daily nursing visits, remote patient monitoring and prescription drug delivery via infusions.
  • Orange, Calif.-based UCI Health signed a deal with virtual care provider Biofourmis to use its AI-powered remote monitoring tools to care for acute and post-acute care patients in their homes.
  • Quincy, Ill.-based Blessing Health System partnered with Biofourmis to launch an acute-level hospital hospital-at-home program. Through the program, Blessing clinicians will visit patients in their homes and monitor them remotely using Biofourmis biosensors and a companion app.
     
  • Decatur, Texas-based Wise Health System signed a deal with Biofourmis to launch a hospital-at-home program. Through the program, Wise Health System will select patients who are medically eligible for hospital admission and provide services such as daily nursing visits and remote patient monitoring.
  • Yale New Haven (Conn.) Health System partnered with Medically Home, a hospital-at-home program, to launch a hospital-at-home program that serves Medicare patients. The program will be a combination of in-person visits and telehealth visits and provide services such as daily nurse visits and remote patient monitoring.
