Patient information data breaches can occur in a number of ways: computer hacking, theft, improper disposal and unauthorized access being a few of them.

Here are the 10 largest reported data breaches due to equipment theft, according to HHS' breach database.

1. Advocate Health (Downers Grove, Ill.): 4 million affected

2. New York City Health & Hospitals Corp.'s North Bronx Healthcare Network (New York): 1.7 million affected

3. AvMed (Miami): 1.2 million affected

4. Blue Cross Blue Shield of Tennessee (Chattanooga): 1 million affected

5. Sutter Medical Foundation (Sacramento, Calif.): 943,000 affected

6. Horizon BCBS of New Jersey (Newark): 839,700 affected

7. AHMC Healthcare (Alhambra, Calif.): 729,000 affected

8. Eisenhower Medical Center (Rancho Mirage, Calif.) 514,330 affected

9. Spartanburg (S.C.) Regional Healthcare System: 400,000 affected

10. Affinity Health Plan (New York): 344,579 affected
